The following criteria must be in place in order to get a quote.

  • The employer has 2 or more full-time employees (minimum of 30 hours a week)
  • Employee/employer applications must be signed and dated within 90 days of the requested effective date for Anthem's quote to be firm
  • The employer must pay 50% or more of the insurance premiums
  • At least 50% of full-time employees and 75% of net eligible employees must enroll

Anthem requires an employer application, employee applications, and a census in order to provide a quote.  One of the most important things to remember is all forms must be completely filled out.  The highlighted areas of both applications must be done.  The census must also be completed…please do not print this…the census will print on 152 pages one line each!  The columns in red must be done.  As always, IPEP members will be represented by their local independent insurance agent.  Once the applications and census have been completely filled out and received by IPEP the information is sent to Anthem.  You will receive your quote within 7-10 business days.

Anthem Employee Application Form

Each employee must complete this application form for the 2-50 groups.  All highlighted sections must be completely filled out before Anthem can provide a quote.

Employer Group Enrollment Form

Applications for the employer that must be completed by all groups regardless of how many are employed.  All highlighted sections must be completely filled out in order to receive a quote.

Employee and Dependent Census

The following columns must be completed to get a quote: · Column A, B, D, F, G, H, P, Q, S, T, U, Y, Z, and AA · Please note, there is a tab for dependents on the spreadsheet that must be completed.  Please do not print the census, as it will pint one line per page on 152 pages.

Congratulations on selecting a great health plan! So…what happens now?  The contact person with your organization will receive an email from Capstone Administrators.  There will be a welcome letter attached to the email and will include your medical group number and effective date from Anthem.

This email is VERY important! Please make sure you respond to this email to get the additional information needed on how to use Benefitsolver.

IPEP has partnered with Capstone to administer its health plan that is available to IPEP members.  Benefitsolver is used to process your eligibility and enrollment.  IPEP members are able to manage their health enrollment plan.  Some of the special features members manage are listed below.
